How do you make a user submittable form using PHP and mySQL?

I have a website hosted by justhost and it uses the cpanel.I want to know how to make a form for visitors to put in information and submit, i think you can make it go to a mysql database. If not, is there another way to make a submittable form?
I am wanting to use the post method, i am unsure of how to actually collect the submitted data though. Sorry if i was unclear about that.
