...fish gets hooked? Ive seen people leave their rods positioned upwards while they relax to drink a beer. So whats stopping the fish from getting hooked and then swimming off with the line and the rod??
 
depends on the size of the fish and what kind of fish

they are probably fishing for catfish which are a real lazy fish when hooked, you probably will not even know they are on the line until you decide to drop the beer and take a wiz and check the line on the way over to the tree...

if you are after something more gameish like a shark or salmon you will definitely need to hold on to the pole or you will lose it.
 
fast reflexes or a bite alarm.
some system to allow the fish to take a bit of line until the rod is reached.
check into a baitrunner type spinning reel or a B/C with a click button on the side.
 
If it's on a fork stick or rod holder it will usually just bend if the drag is set right it won't get pulled in. Unless it's a monster size fish then it's possible I've seen rods snatched in the water. A veteran fisherman is always paying attention to his line though it may not look like it. PVC pipes are the best when fishing on beaches and rivver banks cause the rod handle fits completely down inside the pipe. But most times the rod will bend and give you time to react.
