How do I write a letter to a judge asking for an extension?

I need to write a letter to the judge asking for an extension on the conditions that were placed on me when I was "sentenced". I completed nearly all that I was asked to do, but the evaluation I need to do won't be finished until February, and I only have until December 18th to finish all my conditions. I spoke to my lawyer and he said the only thing I could do was write the judge a letter. I have no idea, first of all, how to format it, and more importantly, I do not know what to tell her.
