How do I make a test strip out of a 3 x 5 photographic print I have?

Call it cheating, but the nearest darkroom to my city is very far; therefore, using Photoshop (or another alternative) I would like to print out a test strip using the 3 x 5 print I have for an assignment that is due soon.

I do have a scanner; I just need to know how to properly resize it according to a test strip and adjust the colors to some black, some white and some gray (again, like a test strip).
