How do I help a student who's afraid of her own reflection?

I've been a teacher for nearly six years now. This year we had a number of new students in our 8th grade, since our school is a carter school it was a surprise to us teachers. There's a girl who's in my 3rd hour rotation and recently I've been getting more and more concerned about her. During early September I started to notice she was quiet and many of her classmates would tease her about the way she wore her uniform, her hair, and just how she acts. It wasn't long after I noticed she started to wear more bracelets on one of her arms, then I started to notice scars on both arms. I figured she had started cutting, but some scars look as if they've been there for a while. Just recently she's dropped a lot of weight and I'm afraid she's starting a fight with anorexia. I know she visits with our consuler on a weekily bases, but I'm afraid it's not enough.

I also know she's been having some hard times with her family, her older brother died recently and her parents are now getting a divorce. I think this is becoming a fatal mixture since she's getting picked on more at school, as a school we're doing more to try and prevent further bullying, but as of now I'm afraid it maybe too late for her. I really need to know how to help her without causing more pain in her life, I wish I could know what to do, but in all my years of teaching I've never met a girl anything like her.
