How do I fix a clutch problem on a Mazda 96 B3000 (Ford Ranger V6) 4×2?

I've replaced the master cylinder, pulled the transmission and replaced the slave cylinder, and replaced the master cylinder again. I did NOT "bench bleed" the master cylinder. I've bled the system again and again (installed) and the gears keep grinding with the clutch pedal on the floor. I'm not feeling any significant resistance on the clutch pedal until it is nearly ½-way down. Please Help!
 
This slave cylinder has to be pumped up after bleeding. So depress the clutch pedal with full strokes (about 50 if I remember right) till it gets extended enough to be in adjustment.
Also there is a residual pressure valve some place in the system that may have failed (in my Bronco II it was in the clutch line).
