How do I create a healthy, natural eating and exercise routine that will burn fat and...

...build muscle? First of all, does this sound like a good meal plan?
Breakfast: oatmeal (w/ water, not milk)
an apple
Snack: a small portion of almonds
Lunch: oat bread (2 slices) w/ turkey, ham, lettuce
non-fat yogurt
carrots
Snack: an apple or banana
Dinner: chicken (grilled or baked)
a vegetable
And this for working out?
Cardio 30 - 60 minutes daily; strength workout 3x a week

I'm trying to get muscular/defined and lose fat around my stomach but I haven't had much success yet. Thank you!
