How do I build a form that will populate rows of an Excel spreadsheet when "Submit"...

...is clicked? (html or .pdf)? We have over 100 paper questionnaires that contain roughly 65 questions on each. I need to import each form directly into an Excel spread sheet (as a new row) so I am trying to create a front end that will allow this.

I can write .html and have the ability to create a form within Adobe Professional 9.0.
