How do I address my letter(inside address in business letter)?

I'm making a business letter but I don't know the right way to address a person in the inside address of the title.
Which is right?
Ms. Juana Cruz
Ms. Juana Cruz, RN, MAN
Juana Cruz, RN, MAN

I was going to use the latter but my classmate said that I must use the second one instead. But it seems wrong for me.
We don't use Ms or Mr. when there's a title at the end, right? Or is it possible to use?


And if not, which is more formal to use?
the title Ms. or her professional title?
