I would only put lamps over the table. I'm not sure what size lamps you will be using but they need to be fairly close together, Maybe 1 ft apart. Mount them at the estimated height that your plants will grow even if you have different areas of your table with the lamps at different heights.
I use potting soil mixed with about 25% top soil from my summer garden. You can go with 100% potting soil if you don't have good top soil. Since you are investing in lights and will be paying extra on your electric bill for heat/electricity, you are right to look for the best.
You may find barnyard fertilize in your area from livestock that are fed only hay and grain. That's my source.
One thing I've started using is big clay pots with heat tape around them, insulating them with news paper, straw, old blankets .. This keeps the roots warm because I don't heat the whole room with anything other than my lights. Your situation may require extra heat. I live in the southern US.
