Step one:
You take the first conversion (1.23 longs = 23.0 mediums) and find out how many mediums are in one long.

That will tell you how many mediums are in 1 long.
Step two:
Then you take the second conversion (74.5 mediums = 645 shorts) and find out how many shorts are in (x)M.
645S over 74.5M is equal to (r)S over (x)M
(xM is your answer from step one)
(r)S is the answer.
