Here is the situation? I have checked my brake and accelerator pedal position and figured that out the accelerator pedal is way far ended right and brake pedal is almost 2 inches closer to my right foot at the time that I remember of hitting the brake pedal I can remember it took more time to press my foot on brake pedal which it was too late to stop the car and rear ended the front car. As a result I have checked that if you wear shoes with wither toe it can easily stuck to the brake pedal's behind so prevents you to hit the brake on time. Also the floor mat is never stay in the place it should be and always moving to the right side of the floor. Have you ever heard any issue regarding that matter?
