Help with programming php/mysql?

I need to create something so that people can come to my website and make a username and a password. Everything I have found on the internet shows me how I have to make a user name and password in the database. How can I make it so that people can make their own username and password and enter it on my database. Just like you would on any other website, youtube, myspace, facebook, yahoo answers. Thanks a lot
