I"m trying to fill out the information best as I can but I'm confuse.
There's a selection that says
Your monthly deductions For: then under it, it says "Union Dues"
What is that?

do you pay union dues? if you do enter the amount you pay.

They're the fees you pay to the workers union at work, if your in one.
