Help, don't know what this is

For the last few weeks I've had this weird feeling. Sometimes it feels like someone is grabbing the side of my face and neck and pulling down on it. Or that I have some heavy bricks being put on my shoulders and they keep getting heavier and heavier.
Some other symptoms I have are: pain in my upper left arm - looks a little swollen today, it's very tender to touch. Pain in my front of my upper legs right above my knees. Feels like my muscles are tightening up. Sometimes I get leg cramps in my calves too. Pain in my knees and sometimes ankles.
I have some back problems - degenerative disk in my upper back and a slight curve in my spine.
What I don't know is what is causing this weird pulling feeling and all the muscle pain and cramps.
I've had an ENT tell me I may have TMJ and a messasge therapist told me she thinks I have fibromyalgia. But I haven't been actually diagnosed with these. Do these cause these types of symptoms?
I feel like I need to see a Dr but I don't know what kind of Dr I should see. I don't know how to discribe the symptoms and it sounds weird when I discribe it.
I've been eating a lot of chocolate lately (from Valentine's Day) Does chocolate cause muscle pain or spasms?
Should I see a Chiro, nerve, muscle, acupunture? I don't know what to do.
Any help will be appreciated. Thank you.
