Health Insurance premium question...?

I am having to switch to my husbands insurance but it seems like the rates are soo high!! It is all a bit confusing though. Maybe someone can clear this up for me. Under the prices for the insurance there are 3 columns:

Maximum plan year rate - $1186
Bank Resp. - $400
Employee Resp. - $467

Does this mean our monthly premium responsibility is $467 but we won't exceed $1186 for the year? My husband can't get a hold of HR until next week so I'm wondering if maybe someone here might know the answer. Thanks for any help!!!
 
I'm not sure what those numbers are because they seem so arbitrary and unlike any insurance program I've dealt with. Usually you will see a monthly premium for having the policy and then a deductible per year. Meaning you pay X per month and the benefits don't start to kick in until you've met your annual deductible. My family plan is $160/month with a $500 annual deductible. It sound like yours is $467/month with a deductible of $1186 before the benefits kick in.
