Gym contract: Social Security # left blank?

I joined a gym a few years back and they have sent me to a sleazy collection agency trying to collect over $1,000 for a gym contract. Not only did I not receive the gym services, but I also moved out of state at the time which would have qualified me to breach their contract. The collection agency says they have a copy of the contract, but refuses to fax it to me. They will only mail it to me, which allows them to skip trace. I asked the lady repeatedly to fax it to me and she refused, yet she said she was looking right at it. #1 I think they're trying to overcollect and #2 I recall never writing my Social Security # in on this gym application. Therefore, it's an incomplete application. Nevertheless, I think something is up, because what collection agency wouldn't readily FAX YOU THE CONTRACT if they're trying to collect over $1,000 for it???
