Through it all you were there for me,
When I had a fever of a hundred and three,
When my mom died and I cried for days,
You were there to pull me out of the haze,
You were there to remind me, that I had you,
No matter how bad I felt, or how blue,
That you were there no matter what we faced,
And I know that you can't be replaced,
And now to finally say words long overdue,
It's always been like this, I love you.
 
Perfect. Consider adding to the last line "It's always been like this, I've always loved you."

It flows a teensy bit better, but it's honestly the best poem I've ever seen that's not in a book, and I hate poetry, so, awesome job.
