Financial aid for lap-band surgery/weight loss centers?

For the past five years, I have tried very hard to lose weight. A myriad of health problems (high blood pressure, a history of heart problems, acute asthma, depression and autism-related seizures) have made it impossible, even when I follow a doctor directed diet and exercise program. Five months ago, another heart problem cost me my job, and left me homeless and without insurance. While I now have a place to stay, I am still having a lot of health problems that are tied in with my weight. I have a disability application pending (due to my autism, not my weight), but have been told it may take another year before I have insurance, and before that is finished processing. At 5' 2" and 210 pounds, my BMI is just barely low enough that I don't qualify under "healthy" guidelines, even if I were to go to a doctor and pay out of pocket. I have been told that I would qualify, however, due to the health problems I have relating to my weight (my blood pressure, severe depression and asthma). Since I have tried everything else, I want to look into the possibility of lap-band surgery. I've done my research, and I would like to find a way to do this. However, since I am unable to work, I need to find a weight loss center, doctor or aid program that might be able to help me out for little or no cost. I'm located in Indiana. I am neither lazy nor a mooch, but I need to get this health problem taken care of before I can deal with some of the others, and I want to be a better mom for my kids.
