Expatriate health insurance?

I'm an American who has had Blue Cross health insurance from the US for several decades. Premiums have increased unconscionably, despite the fact that, fortunately, I have hardly ever had to use it. It also doesn't cover any health care outside the States. What is a reliable and reasonably-priced policy for Americans living in Africa? Thank you.
