Emergency with my angel fish!?

I have a 3yo angel fish that is huge 4-5" tall and he is swimming upside down. What's wrong with him? He lives in a 30gal tank with a balloon molly (aprx 1"), a reg molly (small) and 4 neons. The rest of the fish appear to be doing fine. Two of the neons are fairly new, purchased within the last month, the rest have been with him for a while. I recently purchased a plant for the tank as well. It doesn't appear to be doing well, it is starting to get yellow spots. Should I remove it? How can I save my fish? I'd hate to see him die, I've had him for so long!
pH is 7.2
ammonia is 0
it's a 30gal tank
it has a 50gal Penguin Biowheel 350 filter
tank temperature is 74
I kept the new fish in a separate tank for 2 weeks prior to adding them to the larger tank. They are very small and seem to have adjusted fine (each 1/2" max in length and 1/8" thick). I take out about 5gal of water a week and added 5 new gallons and chlorine treatment.
 
Could you please provide some more information on the tank?

We need information on:

Water parameters (pH, ammonia, nitrite and nitrate)
Tank temperature
Filtration
Cycling (or in other words, did you cycle your tank without fish?)

The more information you can give us, the better we will be able to help you.
