Happy Birthday!
I just wanted to say thank you for everything you have done for me and David. We've all gone through a lot over the past year, thanks to my family. But thanks for always being there, especially for David, and especially for me (even though I'm horrible at giving directions :D) I hope to be able to know you for many more birthdays.

I would suggest you keep out the 'thanks to my family'. Not necessary to bring up unhappy memories. Here's my suggestion which I hope you may consider -

I just wanted to sincerely Thank you for everything that you have done for David and me. We've been through so much this past year. Thanks for always being there for both of us whenever we needed you. You were so selfless. I hope to be able to celebrate many more birthdays with you.
