...documenting your physical assessment? I am in nursing school and my first semester of clinicals, which involves doing a complete physical assessment of the patient. However, I am unsure of how to correctly word my findings and my book does not discuss the terminology in great detail. Surely there is something that can help me!! An example of what im looking for is "The eyes are equal and reactive." That kind of stuff.
