I called the company that I did my Driver's Ed with (an online program) and the woman told me it'd be $25 to replace BUT that I shouldn't need the Driver's Ed form (just the form for training).
She said she thinks they keep it when you take the permit test?? I thought I lost it.
My test is in a week, I need to know pronto! If I have everything else
-actual permit
-proof of driver's TRAINING completion
-parent signature
then do I need the EDUCATION completion form?
