1) From Sabrina The Teenage Witch: A man slips and dumps his coffee into a board held by Sabrina. I think the next thing Sabrina does is reverse time by a bit.
2) From the series premiere of Gotham City: Batman tries to stop an impostor of Ra's al Ghul's decoy on board a Boeing 737-900 but slips and the plane explodes, spreading green Kryptonite across Gotham and mutating several humans, including Claire Bennet and Niki Sanders.
3) From the 4th season finale of Smallville: Jonathan Kent AKA Shape Shifter kidnaps Lana Lang and takes her to a chemical plant. Lana attempts to escape, but she is instead thrown into a vat of chemicals and emerges unchanged, but develops the same powers as Shape Shifter.
4) Two scenes from an unknown Batman movie:
(4-A) Harley Quinn robs a science factory and holds everyone hostage, but is confronted by a pre-Mr. Freeze Victor Fries. Once the two find out that the Joker fell off a building, Fries throws Quinn into a pool of acid, resulting in the creation of Poison Ivy.
(4-B) A revived Rachel Dawes confronts Bruce Wayne in the lawn of Wayne Manor. After throwing Bruce onto a tree, she shows a card to him saying "Ducard" and is revealed to be Talia al Ghul (she was revived by Ra's al Ghul following her death in the explosion in "The Dark Knight").
5) From 2004's Gigaman: Re2urrection: Al Mason is on board a spaceship with an astronaut team of 14. As a cosmic storm with the power of yin and yang approaches the ship, Al enters a pod with the yin and yang shield and blasts back to New York. The entire team fails to escape, and are exposed to the cloud. They make it home intact, however, before long they begin to mutate, developing strange powers. Five of them become evil, while the other nine become heroic.
