Do you know of a free PHP script that can do this?

Hi, I am looking for a script that can do the following:

- Upload about 500,000 names and birthdays to a mySQL database
- Be able to search the database by first name, last name and date of birth
- Users have to login to access search
- Each search and the result is logged in a log file

Also, if you know about an open source software package that can do something similar, I would love to hear about it too.
