Do teachers dicuss how a student is coping in other classes?

Like, if you had a student for a number of years, and then they transfer over to another class the next year, do teachers ever speak to the student's current teacher to find out how they're doing in that class?

Also, can teachers see what grades you got/teacher comments made by other teachers on your report card when they're typing them on the computer?
