What is going on on the cover of this album ("The Sickness"). It looks like a head but with something on it or around it, it is also under the CD holder and its a bigger picture but im not sure what it is supposed to be. Can you tell me what it is and what is going on??

its the bald guys head thats like pushing his head down by his shoulders or w.e.

I think its like a head but with his neck skin or some of his skin pulled up over his mouth to cover it.
