could someone put this address into international mail format?

im trying to get replacement parts from fujimi. i would e-mail them but can't since i dont have a firm grasp of japanese yet. so i decided to write them instead and the address they gave me is very confusing since it's not in international mail format. here's the address they gave me.

FUJIMI MOKEI CO. , LTD. 4-21-1TORO SURUGA-KU SHIZUOKA-CITY, 422-8033 JAPAN

i can only make out the name and postal code but not the street and prefecture/province.
please help.
