could my nieces be taken from my sister in law?

My sister in law has a daughter who is 3 and she is 9 months pregnant with another child that we aren't sure the sex of. she hasn't gone to the doctor throughout this whole pregnancy even once and she did the same thing with our first niece. my 3 yr old niece hasn't seen a doctor since she was about 12 months for any shots or anything like that. she is planning on just showing up at the hospital when she has contractions just like she did with the first one. my question is is it possible that the doctors or family services could take her children from her for not seeing a doctor the whole pregnancy? im just curious as to what happens when the 3 year old goes to school and has no shot records???
she is actually a fabulous mother otherwise. the 3 year old is always clean and she is very smart and well behaved. it doesn't really make sense. but she doesn't go to the doctor i think because she doesn't have insurance and feels like she is above welfare. if my niece was hurting she would take her to the emergency room. she is manic depressant though so that may have something to do with it.
