Cellphone micro sd memory card cant format?

hi, my cellphones micro sd memory card has randomly stopped showing up on my files and it wont format properly, on my phone it says error after trying to format and on the computer it says format complete then when i put it back in my phone it says i need to format it again

on the card it says:

kingston
512MB
MicroSD
0638G54100u
SD-C512 JAPAN
CDC512

Any help will be appreciated :)
