Car/ Bus mode on 18 wos pedal to the medal.?

How to you get the cars and/or buses on 18 wos pedal to the medal? Also, I went into the config and changed the fatigue from 1 to 0 and saved it, but when I went into the game it was still turned on..How do I change it?
