i just watched this movie in health class, but we didnt have to take notes. but, from my research, I can answer question 4.
1.people are denied health care if they have a preexistant condition. (remember the lady with the yeast infection)
2. all rescue workers were denied cae fror condition caused by 911
3. sorry i cant remember a third

an easy way to get the answers is to rewatch the movie from Netfilx or just watch the parts you need help on YouTube.
