the man hasnt gotten it even after years
they only use birthcontrol
why hasnt he gotten it?

Well condoms are birth control, but I'm assuming you mean the pill. In that case, I would say they are having love encounters with other people, not each other.
 
Many STDs are (more easily) transmitted through the mucous membranes of the penis, vulva, rectum, urinary tract and (less often - depending on type of infection) the mouth, throat, respiratory tract and eyes.
She has an STD in her throat, the "MAN" isn't her "HUSBAND" otherwise it would be worded differently, they never kiss because she is married to another man.
