Can an M.D. determine bacterial vs. viral throat infection without a culture?

I came down with another severe tonsil infection over the weekend. One tonsil was more infected than the other, and there were white spots on my tonsils and they were even oozing a little blood. It was accompanied with a high fever and aches and pains. I went to my ENT Specialist M.D. yesterday. I get 2 to 3 of these infections per year, according to my Dr it's not enough to get my tonsils removed. I started myself on some antibiotics on Saturday just incase it was bacterial (Zithromax). Well, he looked in my throat yesterday and said it was bacterial and that he couldn't give me a throat culture because I was already on antibiotics. He prescribed me Augmentin 875mg (Penicillin) twice a day. I'm already starting to feel better on my third dose. The last time I had a similar infection he said it was viral and only gave me Predinsone for the inflamation because antibiotics don't do anything for viral infections. This time he didn't give me that because he said the antibiotics would take care of everything because it was bacterial. My question is can an ENT Specialist really determine bacterial from viral just by looking at my throat without giving me a culture?
