Callous on TOP of hand

I've had this big callous on the top of my hand, on my index finger knuckle. I know callouses are caused by repeated rubbing or gripping something too hard, but how does that occur in a place where that's impossible?

Is there any way to make it go away? I've had it for many years, and it's just kind of unsightly. I've attached a picture.
 
I'm not sure that it is a callus. It could be a fungus or cyst. I would let my doctor take a look before I tried to do anything to it though.

I don't think so, but if you don't do anything to put pressure on that part of your hand, then I can't figure out how it would have developed into a callus. Perhaps you do something that puts pressure on it that you don't realize. To be on the safe side, I would let the doctor look at it.
