Boyfriend has really bad table manners?!?

I have been trying to think of the best way to talk to him about this. My boyfriend's table manners are terrible. I can handle that he stuffs his mouth with huuge chunks of food and finishes a plate in under 5 minutes. (He's military and I assume this is where he gets it because no one else in his family eats that fast) What is driving me crazy are the noises he makes. He eats with his mouth open and is always making this incredibly loud smacking noise. His teeth scrape loudly on his utensils, he slurps soup and even the milk in his cereal. Then picks up the bowl and slurps out the remaining milk. His tongue is always coming out of his mouth to catch all the food or liquid that he spills. He scrapes his fork on his plate all the time and uses his knife to scrape food onto his fork... which defeats the purpose of forks. He doesn't finish chewing before stuffing more food into his mouth. I can't even eat while he's eating because I lose my appetite. Sometimes I have to leave the room. I know this isn't just me. He has a friend at work whose girlfriend I am friends with. Apparently he's been driving people in his shop crazy with his eating habits and the way he chews his gum. For awhile I was kind of hoping someone there would say something to him, but then I realized it may hurt him less if I tell him myself. I know that regardless it's either going to hurt him, make him angry or offend in some way. I just want some suggestions on the best way to let him know he can be more aware of his table etiquette to save him some embarrassment and me some sanity.
