Boosts only for maniacs in mafia wars?

I fought someone then I saw his boost, whoa! a Mr. Hot Sauce which gives a +100 defense skill! Where Can I get it because I'm lvl. 172 mogul and still won the fight!!!!
Is it true that maniacs can only get those boosts?
