Book I can't remember the title of!?

I read it a few years ago when it was new out, it had a green cover with a blurry girl in a white dress on the front. It started with the main character, a middle aged woman, being decapitated with garden shears (cheerful, I know) and being buried down a well, I think.
Any help would be much appreciated!
