Blower motor on may mazda premacy only works on speed 4 ?

As above, it's correctly termed the resistor pack. Some are in easy to reach places, others awkward as hell - try a visit to a dealer.
 
There is a resistor controller unit which will require replacing.
My advice would be to contact a Mazda parts dealership for a replacement and they should be able to tell you where it is located.
They are pretty simple to replace it's just a case of knowing what you are looking for and finding it.
Normally just 2 nuts and a plug to replace the unit.
