I'm creating an asp web form to retrieve information from a access database. I need help generating the code for the steps below: Im new to this so any help would be great!

1. choose an agency (users can choose more than one)
2. Choose from address, address+phone, address+phone+email, or address+phone+email+website
3. Choose state
4: Generate results on page.
