Acromioclavicular (AC Shoulder) separation

Hi All, I was in a car accident involving a semi truck about a week and a half ago. I was blessed to come out okay with the exception of some shoulder pain. I went to the doctor. (had to see the PA, doc was out for the day) She diagnosed me with muscle pain and sent me home with pain meds. (I am highly allergic to anti-inflammatories). They told me to come back in a week or two if I am not better. By Tuesday I could not take the pain anymore so I called the doc and came in on Wed. (it had been 6 days since the accident by now). He immediately looked over the x-rays and stated that I had an AC separation (torn ligament). He prescribed one week off of work (I am a server), limited mobility and some use of a shoulder sling. He also wants me to go to physical therapy. I am to schedule my first appointment tomorrow.

I am also experiencing some pain radiating down my arm, pain in my upper shoulder and into my neck. I am having some numbness and loss of control of my fingers. He mentioned that I could have some damage to my neck area, but it would be only 2 of my fingers that would be useless not all four. I am worried to go to physical therapy. I am afraid that I damaged my arm further since the first set of x-rays and that I will damage it further in therapy. I wake up with little pain but by the end of the day I am in severe pain, even with the meds that he gave me. Does any of this sound familiar to anyone, and do you know whether or not it is normal to have pain all the way down the arm with numbness. Also when I try to lift my arm a certain distance I drop whatever I am holding. For example, I tried to hang some clothing on the rack in my closet and dropped all the hangers before I got to the bar. Any help and advice would be appreciated! Thanks so much!

A little anatomy will help you understand what happened and why you have the problems you do. The AC joint is up on top of your shoulder. It's where the collarbone and shoulder blade come together. Doesn't sound impressive except all the muscles that go down your arm depend on this joint to support them and keep them in place. A torn ligament here will make the entire arm weak. So it doesn't surprise me that you drop things raising your arm.

PT will help to keep you from developing a "frozen shoulder" while the ligaments heal.

Once you heal a little more, you may need to get an MRI of your cervical spine to see if you injured anything there as well. But the symptoms of both injuries as very similar so you need some time to see what gets better. The swelling from the torn AC ligament can cause pressure on the nerves and cause that numbness.

I had to have surgery on both my AC joints as they were close to fusing from arthritis(they cut off the ends of the collarbone). But after surgery, a lot of my pain didn't go away...turned out I had a neck problem too(major arthritis) that no one knew I had. The symptoms are that close.

So give it some time to heal and it at least have the swelling go down. Try ice to relieve the swelling.

If it keeps hurting or you have trouble holding things or you can't write or use your hand, then call the doc back and get that neck looked at. I hope for your sake, it's just the AC joints.

good luck............Jenny
