'98 Toyota 4runner 4x4 Clutch Problem?

I have a '98 4runner 4x4 with a manual tranny. I think the clutch has gone out but I would like to ask the pros on here before I take it into the shop so I can be armed with a little more info.

When you shift gears the clutch doesn't seem to catch. If you give the car any gas at all, it will rev up to the 3-4k rpms and then finally catch. Also, if I am just driving in like 3rd or 4th gear I can press hard on the gas and the engine will rev, almost as if the car is in neutral. If I am very gentle on the gas when shifting it seems to be fine. Also seems to be fine in 1st or 2nd gear. It has progressively been getting worse over the last month.

With the problem I just described above--would you say I need a new clutch or could it possible be something else?
 
It does sound like a clutch problem but it could be for several reasons. If you have any ol leaks, they could contaminate the friction material, also its possible for the clutch to be out of adjustment, or it could simply be tour clutch is worn out.
 
You need a clutch a bad slave cylinder would not make it slip only having a worn out clutch disk and pressure plate.
