92 jeep cherokee has lost alot of power.?

If i accelerate too quickly or going up hill, the rpm's will jump and wont let me have the power i need. Seems to not want to hit the next gear. This is really bad when im going up hill. Flat ground or down hill seems to have not too bad of a problem, just when i give it too much gas.
