91 ford ranger lacks acceleration...?

I have a 1991 ford ranger with 180 thousand miles. I recently acquired the truck so i dont know the full history, or how long its had this problem. but the problem is that it seems to lack considerable acceleration. You notice this especially when starting out in first and second gear, and even in third. 4th and 5th gear the truck seems to run just fine, however i should mention that it wont really go much faster than 65 mph without hitting 2500 rpms. (the speedometer only goes to 85, so im not expecting much better there...)

I know its an old beater, however this really concerns me when im on the highway. It will run fine until i start going up a hill, then no matter how much gas you give it it will just start slowing down, more and more until you have to shift down to fourth and even third gear, going 35 to 40 mph in a 65 zone.

Any thoughts on whats causing this?

oh and i just changed the air filter so i know thats not an issue...
