2001 Ford Mustang manual transmission acting strange.?

So I have a 2001 Ford Mustang (V6) manual that recently starting acting rather odd. Basically, it sounds like it is going to stall out even though the rpms are at the right level, and then the car hops. It seems to do this on most my gear changes, although giving the car a lot of extra gas before a change sometimes seems to reduce the problem. I bought the car new, and it never has acted like this before... it has about 80k miles on it, and the only other thing to note is that the Check Engine light has been on for a while, and when they hooked up computer it read that it was an emissions issue.

Any idea what might be the cause, and any easy fixes. Thanks!
 
I would start off by jacking up both rear tires. Put your tranny in nuetral , and see if there is any noise or tension when you spin the wheels by hand. You may have bad u-joints that with put a strain on your tranny. also make sure your differential and tranny are full of fluid.
