2000 Dodge Dakota 3.9L V6 - Where is my vacuum leak?

Vehicle:

2000 Dodge Dakota
3.9L V6
Automatic 4-speed
108,000 miles
Religiously maintained

Symptoms:

Takes way too long to start when cool/cold.
Hesitates when backing when brakes are engaged.
Hesitates to almost a stall when accelerating when cool/cold.
Late shifting when cool/cold with 3/4 to full throttle. No problem with 1/2 throttle acceleration.
(Sounds like a vacuum leak to me, but if not let me know.)

Parts replaced/things tried:

Spark plugs
Ignition wires
Distributor cap and rotor
Throttle position sensor
PCV valve and hose
Transmission oil and gasket (dropped pan, NOT a flush)
Used Seafoam to clean intake, crankcase and injectors

I have tried spraying every vacuum connection I can see with starting fluid to see if the engine will rev up with no luck. If you know where to find a vacuum schematic for this vehicle, I'd appreciate that as well. Please help me out!
Yep, checked the brake booster. The hoses aren't cracked and pulling each off the check valve causes a sharp surge in RPM which levels off in ~10 seconds. They must still be getting significant vacuum.
