1991 honda civic problem please help !!!!!!?

i have a 91 honda civic hatchback stick shift 5 speed that am having trouble with it. it turn on and runs but when i accelerate like around 4 thousands rpm it hesitates now it runs to but cant get it to go more then 15 mph cause it hesitates it kind of hold back now i shift to 2 gear and same thing what could be the problem please help thanks
