Because it never went away in the first place. Some infection can get deep into the sinuses making killing it off very difficult. It may take multiple rounds of antibiotics. Have you tried a neti pot or sinus rinse? It often can help flush out the infections.

Probably not a big deal. Commercial jets aren't capable of maneuvers that would produce enough g-forces to really mess with you. There are pressure changes during takeoff and landing, though. If you have trouble with your sinuses or inner ear, airplanes can aggravate it. Try chewing gum...
