YouTube Partnership Application - Business information?

Okay, so I'm currently filling out the YouTube Partner Program application and it's asking my for my business information. So company name, job title, company website. It says these are required fields. I'm a cashier at a grocery store.. is that what they want me to put? Or am I supposed to act as if my videos are my "business" and the website is my blog..? I have no idea.

Also, it asks for video content. I upload makeup tutorials, so would I check "Other" and say something like "Beauty" or "Makeup Tutorials" ?
